Instructions: Click on the the Crop Year 2012 Research Reporting Form pdf or word doc. After it opens, click on Save As.
Save it to your desktop. Fill in the required fields. After you are finished, send it as an attachment to Gail Podenski at gpodenski@gvtel.com.

(Reminder - You can not add your information to the Crop Year 2012 Research Reporting Form by using Adobe Reader. You must open it in
Adobe Acrobat to  make your changes. If you do not have Adobe Acrobat, please use the Word Document form.)
